The Most Common Car Injury in America Wasn’t Being Properly Tested Until January
Neck sprains and strains are the single most frequently reported injury in American auto insurance claims, ahead of broken bones, lacerations, and concussions by a wide margin. Whiplash. And for years, the test that was supposed to catch bad seat designs simulated a rear impact at 20 miles per hour, a speed below what most real rear-end collisions on non-highway roads actually produce.[1]
In January, the Insurance Institute for Highway Safety finally added a 30 mph impact pulse to its seat and head restraint evaluation, and the results from the first batch of 18 compact SUVs landed like a diagnostic nobody asked for: four scored Good, nine scored Acceptable, two Marginal, three Poor.[2] Poor performers included the Ford Bronco Sport, Hyundai Tucson, and Mazda CX-50, all popular vehicles that passed previous IIHS evaluations and are sitting in driveways, school pickup lines, and highway on-ramps right now.
IIHS published the crash video, and Poor looks exactly as bad as the word implies. In the Tucson, the head restraint shoves the dummy’s chin down into its chest, compressing the cervical spine in a direction human necks were never designed to fold. In the CX-50, the head slides backward and upward, away from the restraint entirely, as though the seat were designed to showcase the injury mechanism rather than prevent it. Ford’s Bronco Sport takes so long to make head-to-restraint contact that the cervical spine has already straightened and stretched before the restraint does anything at all.[3] You can watch the vertebrae separate on camera, which is not something you want to think about while merging onto the freeway behind a distracted Suburban. In a Good-rated vehicle like the RAV4 or Forester, the spine holds its natural curve and the head barely moves relative to the torso under identical crash energy.
A FARS cross-reference is suggestive but not causal, because whiplash is overwhelmingly non-fatal and FARS only tracks deaths; still, it rhymes. Toyota’s RAV4, rated Good in whiplash, has a fatality rate of 0.19 per 100 million VMT, and Subaru’s Forester (also Good) sits at 0.26. Hyundai’s Tucson, rated Poor, posts 0.34, which is 1.8 times the RAV4 and 1.3 times the Forester.[4] Correlation, not causation, but vehicles designed to protect the cervical spine at 30 mph seem to be designed to protect occupants at other speeds too, because engineering quality tends to be systemic rather than modular.
Give the counterargument its full weight: IIHS’s earlier seat evaluation, introduced in 2003, genuinely worked. Automakers redesigned head restraints in response, and insurance claims data showed real, measurable reductions in whiplash injuries over the following decade.[5] Upgrading to 30 mph is iterative improvement, not an indictment of everything that came before. Fair enough. But 22 years of iteration that left 78% of current SUVs unable to earn a Good rating at a speed most rear-end crashes exceed suggests the iteration crept while the fleet moved on, and the gap between what the test measured and what drivers experienced grew wider every year it wasn’t closed.
IIHS senior research engineer Marcy Edwards, who developed the new protocol, was candid about what remains unknown: “We don’t yet fully understand the mechanisms that cause whiplash injuries.”[6] That honesty matters, because this is a test built on the best available biomechanical research rather than a definitive model of injury, and only 18 compact SUVs have gone through it so far while sedans, trucks, and full-size SUVs wait their turn. Results could shift as more vehicles enter the protocol. But the principle holds: the most commonly claimed injury in American driving was being evaluated at a speed that understated the forces most occupants actually experience, and nobody changed the test until seven months ago.
What to do with this: If you’re buying a compact SUV, check the IIHS whiplash prevention rating at iihs.org/ratings before signing, because the RAV4, Forester, Ioniq 5, and Audi Q3 earned Good while vehicles with twice their fatality rate earned Poor. If you own a Tucson, CX-50, or Bronco Sport, adjust your head restraint so the top is level with the top of your head and as close to the back of your skull as possible; it won’t fix the underlying seat design, but geometry still matters when the car behind you isn’t paying attention.
