NHTSA's Budget Went Up 459%. Traffic Deaths Dropped 16%. The Automakers Did the Rest.

A new peer-reviewed study in Traffic Injury Prevention does something nobody at the Department of Transportation will thank the author for: it puts numbers on NHTSA's futility.[1] Since 1979, the United States has cut traffic fatalities by 16 percent. Fourteen comparable high-income countries averaged a 77.4 percent reduction over the same period. The gap has been statistically significant since 1996 and it has widened every single year since then, without exception, for three straight decades of divergence while NHTSA's Congressional appropriations climbed by a factor of nearly five.

David Viano, the biomechanics researcher who wrote the paper, identified twelve specific institutional failures ranging from the absence of fatality-reduction targets to crash tests that bear no relationship to the collisions that actually kill people. But the finding that should keep administrators awake is simpler than any of those: fatalities in the United States would have increased over the past quarter-century if not for safety technologies that automakers introduced on their own, voluntarily, without waiting for the regulator to mandate them: Electronic Stability Control, Automatic Emergency Braking, and high-retention seat designs, the three technologies that account for virtually every measurable gain in American road safety, arrived because manufacturers and the Insurance Institute for Highway Safety pushed them into production while NHTSA spent its growing budget on test protocols that, per the study, have "no relevance to fatal accidents."[1]

The agency's current budget sits around $1.3 billion a year.[2] Its staff has hovered near 675 employees for years. And the fatal-crash population it is nominally protecting the public from consists overwhelmingly of people driving vehicles more than ten years old, unbelted, often impaired, frequently speeding, in cars that will never see the inside of NHTSA's New Car Assessment Program lab. FARS data confirms the pattern independently: one in five drivers in fatal crashes tests positive for alcohol or drugs.[3] The people dying are not driving 2026 models with five-star ratings. They are driving 2012 Impalas with a blood-alcohol level north of the legal limit. NHTSA has no program for that fleet, no mandate, no test.

Meanwhile, other countries spent the same decades building the infrastructure NHTSA never attempted. The CDC quantified part of the result last year: between 2013 and 2022, American pedestrian death rates rose 50 percent while peer nations cut theirs by a median of nearly 25 percent.[4] The United States had the highest pedestrian fatality rate among all twenty-eight countries studied: highest among teenagers, highest among working-age adults, highest overall. This is a country that tells itself it leads the world on automotive safety because it grades new cars on a five-star scale that, as Viano documents, doesn't predict whether anyone will survive a real-world crash.

Viano's conclusion is blunt: "NHTSA must change their leadership, because the Agency has failed its core mission to reduce traffic deaths the past 25+ years." He does not accuse NHTSA of corruption or bad faith, which makes the indictment worse, because what he describes is irrelevance. A regulator that tests new vehicles in configurations that do not match fatal crashes, publishes scores the public trusts but that do not predict outcomes, and spends fourteen percent more money each year without setting a single fatality-reduction target it can be held to. The automakers, incentivized by litigation, insurance data, and IIHS public ratings, did the actual work while NHTSA took the credit.

What you should do

Safety tech saves lives, but only if it's in your car. If you're driving a vehicle older than 2012, you almost certainly lack ESC, AEB, and lane-departure warning. Those three features account for most of the measurable fatality reduction in the United States over the past two decades. Check what your vehicle actually has, not what NHTSA rated it. If you're shopping, IIHS Top Safety Pick+ ratings predict real-world outcomes far better than NHTSA's five-star system. And if your vehicle is under recall, check your VIN at nhtsa.gov/recalls, because NHTSA does manage to maintain the recall database, even if it can't seem to do much else.

Limitations

Viano's paper is a single-author analysis published in a respected but not top-tier journal. The 459% budget figure aggregates multiple funding streams and may include highway safety grants to states, which complicate a direct comparison to NHTSA's core regulatory operations. The international comparison uses national-level data that does not fully control for differences in road design, healthcare access, vehicle fleet age, or driving exposure. And the claim that fatalities would have risen without voluntary manufacturer technology is a counterfactual that depends on modeling assumptions the paper does not fully disclose. None of this erases the core finding: a fivefold budget increase and a 16 percent fatality reduction is indefensible by any modeling assumption. But it means the precise magnitude of NHTSA's failure may be arguable even if the direction is not.

The strongest counterargument

NHTSA's defenders will say the agency's state grant programs, which fund sobriety checkpoints, seatbelt enforcement campaigns, and Click It or Ticket initiatives, constitute its real contribution to fatality reduction and are not captured by Viano's analysis of vehicle-level crash tests and research priorities. That argument has some merit: states that run aggressive impaired-driving enforcement do show lower fatality rates, and those programs are partially federally funded. But if the best case for a $1.3 billion regulator is that it distributes grants to state police departments, the question becomes whether a regulatory agency is the right entity for that function, or whether the money would save more lives somewhere else entirely.
